Position Description

Position:  Community Wellness and Coordinated Care Director

Exemption Status:  Exempt

Reports to: Community Information Officer

Salary: $87,500-$100,000 DOE – Depending On Experience

+ $2080 – 24/7 on-call

Salary Incentives (annual):

+ $1040 certified bilingual

+ $2080 Inform USA Certified

Summary: Responsible for overseeing and managing coordinated care and Enhanced Care Management (ECM) programs, ensuring high-quality care coordination for high-need clients. This role involves leadership, strategic planning, and operational management to meet program goals and comply with Department of Healthcare Services (DCHS), Managed Care Plans (MCPs), and other contractual requirements.

 

Key Responsibilities:

 

  1. Program Leadership and Management:
  • Oversee the overall implementation and operation of coordinated care programs providing direct case management to high-acuity clients
  • Develop and implement strategies to achieve program goals and improve client health outcomes
  • Manage and mentor the Coordinated Care team including Managers, Care Coordinators, and other staff
  • Collaborate closely with the Clinical Consultant to ensure all care plans meet clinical standards, integrate best practices, and address complex medical needs effectively
  • Ensure compliance with all requirements outlined in the Enhanced Care Management (ECM) and other provider agreements, including reporting, data sharing, privacy/security standards, quality assurance measures, and client care procedures

 

  1. Quality Assurance and Performance Improvement:
  • Monitor and evaluate program effectiveness and quality measures
  • Develop and implement quality improvement initiatives
  • Analyze data and prepare reports on program outcomes and performance
  • Oversee the development and implementation of policies and procedures to prevent fraud, waste, and abuse

 

  1. Stakeholder Engagement and Collaboration:
  • Build and maintain relationships with MCPs, healthcare providers, community organizations, and other stakeholders
  • Manage relationships with Health Plans, DHCS, and other stakeholders as required by the contract
  • Represent the ECM and Coordinated Care programs in meetings and collaborative efforts

 

  1. Financial Management and Compliance:
  • Oversee program budget and resource allocation
  • Maximize billable services while ensuring appropriate utilization
  • Ensure adherence to the non-discrimination and cultural competency requirements outlined in the contract and agency policies

 

  1. Systems and Reporting:
  • Oversee the implementation and maintenance of coordinated care management documentation systems that meet the requirements specified in the contracts
  • Ensure timely submission of all required reports, including staffing and capacity reports

 

  1. Continuous Improvement:
  • Stay informed about changes in policies, regulations, and best practices
  • Implement new requirements and adjust program operations as needed

 

  1. Agency:
  • Stand in for the Community Information Officer as needed
  • Participate in Executive Team Activities
  • Other duties as assigned

 

 

Qualifications:

 

  • Bachelor’s degree in healthcare administration, public health, social work, or another related field preferred; M.A. desirable.
  • 5+ years of experience in management, preferably in care coordination or case management
  • Strong understanding of Medi-Cal, managed care, and population health management
  • Excellent leadership, communication, and interpersonal skills
  • Ability to analyze complex data and translate insights into actionable strategies
  • Thorough knowledge of applicable state and federal healthcare laws and regulations
  • Successful background and criminal clearance required and maintained, including but not limited to California Department of Justice (DOJ), Federal Bureau of Investigation (FBI, Child Abuse Index and Department of Motor Vehicles (DMV).  Proof of valid California Drivers License, reliable transportation, proof of automobile insurance (listed as covered), pass and maintain driving approval as required by our insurer.
