Position Description

Position: Associate Director, Community Wellness and Coordinated Care

Exemption Status: Exempt

Reports to: Director, Community Wellness and Coordinated Care

Salary: $83,200 / year

Salary Incentives (annual):

  • $1040 certified bilingual English/Spanish
  • $2080 Inform USA Certified

Summary: Responsible for managing daily operations of community wellness, coordinated care and Enhanced Care Management (ECM) programs, with a focus on program integration, technology implementation, and quality assurance. This role oversees multiple Care teams, ensures contractual compliance, and develops systems to support efficient program operations and billing processes.

Key Responsibilities:

  1. Operational Leadership and Team Management:
  • Directly supervise Care teams, providing guidance and support for client care delivery
  • Manage team caseloads and monitor service delivery to ensure contract compliance
  • Oversee staff training and development programs
  • Ensure teams meet required client interaction frequencies and documentation standards
  • Monitor and evaluate team performance and implement improvement strategies
  1. Systems Development and Integration:
  • Lead the development and implementation of technology systems for program delivery
  • Integrate contract requirements into existing agency programs and processes
  • Design and refine workflows for client intake, service delivery, and documentation
  • Collaborate with Technology program to ensure systems meet program needs
  • Develop and maintain efficient billing and reporting processes
  1. Quality Assurance and Compliance:
  • Monitor data quality and documentation completeness
  • Ensure all client interactions are properly recorded and meet contract requirements
  • Develop and implement quality improvement processes

  • Work with Director and Clinical Consultant to review and enhance care plans
  • Monitor and report on program metrics and outcomes
  1. Program Development:
  • Support program growth from initial implementation through full operation
  • Lead specific initiatives related to technology integration and process improvement
  • Assist in developing and implementing program policies and procedures
  • Contribute to strategic planning for program expansion
  1. Stakeholder Engagement:
  • Assist Director with community outreach, advocacy and civic engagement activities
  • Represent the program in external meetings with contractual and community partners
  • Build and maintain positive relationships with internal program directors and staff
  • Facilitate communication between program teams and external partners
  1. Financial Management Support:
  • Monitor service delivery to ensure billing requirements are met
  • Assist Director with budget development and management
  • Assist in financial sustainability activities through resource generation and development
  • Ensure efficient billing processes through proper documentation and data management
  • Track program metrics that impact revenue
  1. Agency Responsibilities:
  • Share 24/7 on-call responsibilities with Director
  • Participate in agency leadership activities and strategic goals
  • Other duties as assigned

Qualifications:

  • Bachelor’s degree in public health, healthcare administration, social work, psychology, or
  • related field preferred
  • 4-5 years of management experience in public health, social services, or related field
  • Experience managing teams and complex programs
  • Strong understanding of healthcare systems and social services
  • Excellent project management and organizational skills
  • Demonstrated ability to develop and implement new systems and processes
  • Strong analytical and problem-solving skills
  • Experience with healthcare documentation systems and data management
  • Excellent communication and interpersonal skills
  • Ability to work effectively with diverse stakeholders
  • Successful background and criminal clearance required and maintained, including but not limited to California Department of Justice (DOJ), Federal Bureau of Investigation (FBI),
  • Child Abuse Index and Department of Motor Vehicles (DMV)
  • Proof of valid California Driver’s License, reliable transportation, proof of automobile insurance (listed as covered), pass and maintain driving approval as required by our insurer
  • Bilingual Preferred: upon passing certification, preferred for potential daily use

The ideal candidate will have strong operational and analytical skills, combined with the ability to build and maintain relationships with various stakeholders.
