Youth Outreach Specialist (I, II, III)

Hourly Rate: $20 (I), $21(II), $22 (III)/hr + $1/hr with bilingual certification
Office Locations: Camarillo and Oxnard
OVERVIEW:
This position falls within the Case Manager job classification. Reports to the Youth Outreach & Homeless Services Program Manager and constitutes a member of the Youth Crisis & Homeless Services department. Provides trauma informed, client centered, culturally and linguistically appropriate advocacy, case management and supportive services for youth, ages 12-24, experiencing or at-risk of experiencing homelessness. Responsibilities include engaging homeless individuals via street outreach activities; coordinating referrals and program linkage to other regional programs serving homeless individuals; maintaining thorough and accurate records in both written form and through the Homeless Management Information System (HMIS); and collaborate with other homeless outreach entities in the Continuum of Care who are part of the Coordinated Entry System (CES).
DUTIES:
- Conduct in-person intake and assessments and provide comprehensive case management and support to clients
- Coordinate referrals and linkages to other community resources
- Provide or facilitate client transportation to and from appointments
- Serve as liaison with community partners and volunteers
- Complete and submit timely documentation on each required service according to contract and/or program requirements
- Develop a goal-oriented care plan, monitor client’s response to interventions and update or modify plans as needed
- Facilitate psychoeducational groups for clients
- Coordinate and attend community outreach events
- Develop and maintain collaborative partnerships with public and private community agencies
- Facilitate presentations for communities and partners
- Maintain up to date Street Outreach Team’s logs and activities.
- Requires support monitoring youth outreach line.
- Assess youth and family needs during crisis and determine need for temporary emergency shelter, provide emergency field response as needed.
- Support development of the department’s social media outreach content.

QUALIFICATIONS:
Associate degree or 2 years equivalent work/volunteer/relevant lived experience. Bachelor’s degree in a related field is preferred. A minimum of 3 months’ experience in social service or customer service setting (I) OR 1 year of experience providing case management (II) OR 2 years’ experience providing case management to the target population or a similar group (III) is required. Relevant lived experience preferred. Must be sensitive and competent to serve clients from diverse cultures and languages. Must possess ability to handle emergency/crisis situations, as well as strong decision making, case management, conflict resolution, leadership and excellent time management skills. Must be comfortable and proficient in using electronic health record (EHR) systems for data entry. Must be able to prioritize tasks, organize thoughts and communicate well both verbally and in writing. Must have completed or be willing to complete motivational interviewing, and trauma informed approaches trainings within 3 months of hire. Requires ability to collaborate with agency staff and community partners while demonstrating exemplary customer service. Must be computer literate with Microsoft Office and the internet. Must be familiar with mandatory child abuse reporting requirements.
Successful background and criminal clearance required and maintained, including but not limited to California Department of Justice (DOJ), Federal Bureau of Investigation (FBI, Child Abuse Index and Department of Motor Vehicles (DMV). Proof of a valid California Driver’s License, reliable transportation, proof of automobile insurance (listed as covered), pass and maintain driving approval as required by our insurer. Ability to comply with all Community Care Licensing Standards which includes Proof of current (within 1 year) negative TB test, or willingness to obtain one. Ability to obtain a physical to perform work duties. Valid First Aid and CPR certification or willingness/ability to be certified.
