Position: Referrals Coordinator II – Community Wellness & Coordinated Care

Hours: Full Time

Hourly Rate: $21/hour

+ $1 Bilingual incentive upon passing written/verbal certification exam

+ $1 Inform USA incentive upon passing certification exam

DESCRIPTION

Reports to the Associate Director. Responsible for initial and follow-up contacts with individuals and families requesting coordinated care services, ensuring program eligibility, performing initial intake and screenings, referring to most appropriate care teams, scheduling follow-up assessments, quality review, and accurate documentation. Track and maintain necessary data in department systems. Provide general office support for the Community Wellness & Coordinated Care team.

DUTIES

  1. Provide timely program information using excellent customer service skills.
  2. Complete eligibility screenings, collect client data and input into electronic systems, review and verify eligibility for services.
  3. Schedule and provide follow-up reminders for assessments.
  4. Follow-up with callers as needed.
  5. Provide information about and refer clients to external agencies as needed.
  6. Effectively respond to challenging conversations and determine when to escalate as needed.
  7. Perform technical quality review of EHR data and documentation according to compliance standards.
  8. Accurately review and complete assigned reports and documents.
  9. Maintain positive internal and external relationships with agency providers and community partners.
  10. Liaise with program team to ensure accuracy of Scheduler and Calendar.
  11. Comply with HIPAA standards and maintain confidentiality.
  12. Attend supervision, agency, department, team, and training meetings as requested by management.
  13. Maintain lines of communication with supervisor and other program staff.
  14. Assist with gathering supporting documentation or information to submit service authorizations.
  15. Foster a positive working environment by upholding the agency’s mission, vision, and values in all interactions and activities, demonstrating a team-oriented spirit.
  16. Fulfill mandated reporting requirements.
  17. Provide office support for department needs as requested.
  18. Other related duties as assigned.

QUALIFICATIONS

  • Bilingual (Spanish/English) oral/written certification exam required (must pass in order to remain employed in this role)
  • 1-2 years of experience in the fields of social services, mental health, medicine, or case management desired.
  • Utilize trauma informed skills to complete screenings and engage with diverse populations seeking case management services.
  • Demonstrate quality customer service skills with internal and external stakeholders, collaboration skills, and attention to detail.
  • Advanced phone and organizational skills completing work thoroughly and accurately.
  • Computer literate with comfort and ease using Microsoft Office products with intermediate to advanced Excel and electronic health record experience.
  • Must demonstrate a solid work ethic, flexibility, and ability to work independently as well as participate as part of a multidisciplinary team.
  • Successful background and criminal clearance required and maintained, including but not limited to California Department of Justice (DOJ), Federal Bureau of Investigation (FBI, Child Abuse Index and Department of Motor Vehicles (DMV).
  • Proof of a valid California Driver’s License, reliable transportation, proof of automobile insurance (listed as covered), pass and maintain driving approval as required by our insurer
