- DESCRIPTION OF SERVICES: Transitions is a Behavioral Health treatment program that serves young adults between the ages of 17 ½ to 25. Program objectives are to coordinate agency partners to assist the youth and family members in developing skills necessary for independent living; developing a network of social and community supports; reducing psychiatric symptoms and substance use that negatively impact life functioning. Transitions offers a full range of behavioral health services including assessment, medication management, group therapy, dual diagnosis treatment, rehabilitation services, case management, independent living skills training, family support and education, linkage to employment and housing, individual treatment.
