• DESCRIPTION OF SERVICES: State licensed, Medicare and MediCal certified, and JCAHO accredited, TLC Home Hospice offers physical (pain control), emotional, spiritual and social support services. With a team approach, nurses, home health aides, physicians, therapists, social workers, clergy and trained volunteers meet regularly to plan the best hospice care. TLC teaches the family how to care for the patient, make regular home visits, provide caregiver relief and help determine needed services. TLC also provides services in long term care facilities to eligible patients. Bereavement services continue to assist families for approximately one year after death. Care is delivered by a team of professionals and can generally be started within 24 hours of notice.
