- DESCRIPTION OF SERVICES: Transitions is a Behavioral Health treatment program that serves young adults between the ages of 17-1/2 to 21. Program objectives are to coordinate agency partners to assist the youth and family members in:
1) developing skills necessary for independent living;
2) developing a network of social and community supports; and
3) reducing psychiatric symptoms and substance use that negatively impact life functioning.
Transitions offers a full range of behavioral health services, including: assessment, medication management, group therapy, dual diagnosis treatment, rehabilitation services, case management, independent living skills training, family support and education, and linkage to employment and housing
