• INTAKE PROCEDURES: Visit Nurse Practitioners at published sites during hours scheduled. Must certify as homeless.
  
• SPECIAL ITEMS: Wheelchair access at most sites; Spanish
  
• DESCRIPTION OF SERVICES: A Nurse Practitioner assesses medical needs of homeless or transitionally homeless patients at various networking sites on a regular schedule. Medication access available and referrals are made for follow-up services. Immunizations, TB testing, primary medical care, etc. are available.
