- DESCRIPTION OF SERVICES: CHDP is a state and federally funded program that provides well-child care for qualified infants, children and adolescents. Well-child exams include physical exam, dental and nutrition evaluation, health education, hearing and vision screening, testing for anemia, diabetes, TB and lead poisoning and all immunizations. Services are provided by over 50 enrolled private and public physicians. The purpose of the program is to provide for the early detection and prevention of disease in children. If a condition is discovered, referral and follow-up assistance is provided. Funding for treatment for Medi-Cal clients is through the Medi-Cal program. Eligibility for those without Medi-Cal is determined through the CHDP Gateway to Health insurance which can be accessed through the CHDP provider's office. Clients have access to temporary Medi-Cal eligibility while they complete the enrollment process for Medi-Cal or Healthy Family. Health insurance programs are available.
