• DESCRIPTION OF SERVICES: Provides occupational and physical therapy to medically eligible children from 0-21 years of age. Eligibility criteria requires physician medical reports to substantiate the neuromuscular condition. "Developmental Delay", without stated physical signs of probable neuromuscular disease, is not an eligible condition. Examples of eligible conditions include Cerebral Palsy, Juvenile Rheumatoid Arthritis, Muscular Dystrophy, Spina Bifida.
