• DESCRIPTION OF SERVICES: Program provides for developmental follow-up for infants who are eligible. Eligibility criteria: birth weight under 1500 grams, assisted ventilation for more than 48 hours, seizure activity within the first week of life, sustained hypoglycemia or hyperbilirubinemia, frequent apnea requiring stimulation. Program covers only infants up to 24 months adjusted gestational age.
